Sloughing Mucosa in Esophagitis Dissecans Superficialis

A 29-year-old man with alcohol abuse presented to the emergency department for multiple episodes of hematemesis. He did disclose epigastric pain and gastroesophageal reflux. was hemodynamically stable hemoglobin 12.8 g/dL. An upper endoscopy revealed a striking sloughing mucosa into sheets casts in distal esophagus (Figures B), consistent diagnosis esophagitis dissecans superficialis (EDS). treated bowel rest acid suppression, his symptoms resolved over 2 days. EDS is rare condition characterized by desquamation associated irritants, autoimmune conditions, certain medications including bisphosphonates nonsteroidal anti-inflammatory drugs. It benign entity good prognosis that usually resolves without any lasting pathophysiological changes. Endoscopists should recognize this endoscopically esophageal appearance. Patients are successfully managed suppression elimination risk factors. be considered differential patients gastrointestinal (especially hematemesis) endoscopic findings esophagitis. Caution taken administration after diagnosis, discontinuation may advised.
